Hartz IV recipients are sick more often

Those receiving Hartz IV benefits are more likely to be ill, according to a study by the Allgemeine Ortskrankenkassen (AOK) Rhineland / Hamburg. According to this, the social status of people with statutory health insurance is an important factor for the health of the insured. But why is it like that?

Bad health condition

The health status of many Hartz IV recipients can be described as bad. A study by the AOK showed that financial and social status are closely linked to health. This is particularly evident in cardiovascular diseases and chronic metabolic disorders. "Financial status has a huge impact on general health for adults and children," the study says.

The data collected showed that 6.8 percent of the workforce suffer from coronary heart disease. Among the Hartz IV relatives, there are a whopping 10.5 percent of the insured who have narrowed coronary arteries. It looks similar for type II diabetes. Here 5.5 percent of the employees are ill, but 9.1 percent of the unemployed.

Swapped cause and effect?

Many will object that the cause and effect could be reversed. For example, that sick people get Hartz IV because of their suffering. However, the report shows that Hartz IV relatives are simply under-served due to their social status. For example, 70 percent of working mothers have a midwife come home after the child is born. According to the report, the number of Hartz IV relatives was just under 33 percent.
The differences are also more than clear when it comes to dental health. In 78 percent of children of insured workers, at the age of six, they had teeth without fillings. The figure for children in Hartz IV families is only 68 percent. If you look at the children of those who are voluntarily insured by law (annual income at least: 60,750 euros), the differences are even more serious. Here 83 percent of children up to the age of 6 have no tooth fillings.

Differences from young to old

The differences in health care can be read from childhood to old age, according to the AOK report. Men who have EUR 800 or less per month need a care level at the age of 74. With an income of 800 to 1600 euros per month, the average age for a care level is 77 years. People who have a pension of EUR 1,600 or more only become nursing care on average from the age of 81.

The Robert Koch Institute already stated: “People with low social status are increasingly affected by chronic diseases, psychosomatic complaints, accident injuries and disabilities. They rate their own health less well and report health-related restrictions in everyday life more frequently. ”

Equal care for everyone - but why are Hartz IV relatives sick more often?

What are the specific reasons? Basically, compulsory insurance should apply to everyone equally. However, brain research confirms that a so-called needs pyramid exists. The organism first demands to satisfy existential needs: These include hunger and thirst, sleeping and breathing. The latter is so elementary that we cannot control it at will: if we force ourselves to hold our breath, the pressure will eventually become overpowering to breathe in.

If we starve and freeze, we can suppress this for a certain time by concentrating on the spiritual, but this need comes to the fore and displaces all secondary needs.
These primary needs are immediate, pushing for acute satisfaction. More abstract needs recede behind them. Precautions are not immediate: it's about the future, so important as health is, it's about an abstract concept.

Surviving in the present instead of living in the future

Those who live from hand to mouth in the literal sense of the word care little about how their health will look in the future.

Cancer and general disease prevention are part of the life planning model. But whoever is struggling to get basic food in the last week of the month, who is afraid to freeze in winter because he cannot pay for the heating, whoever the sanctions of the job center take away the opportunities to get the essentials - he lacks the freedom to plan his life.

Social security instead of an index finger

In other words: Those who live on Hartz IV struggle to meet their primary life needs. So if you want the unemployed to take part in preventive medical check-ups more often, you have to get involved in social protection for those affected. (sb)
